Today's health insurance paradigm is often overwhelming!

Navigating through the volumes of information on the internet or what you receive from insurance agents can seem like a real challenge. From selecting a medical insurance provider, to what type of insurance plan, or to coverage limits that best suit your needs and budget...all while aligning the type of plan with a healthcare provider you trust...can be a bit much to process (or afford!).

For individuals without insurance, high deductible health plans, or even those with Med-Share programs, we offer several options to help cover expenses linked with services rendered through AFH.

Options to pay for services rendered through AFH:

1...Use your insurance. If we file for services using your provided medical insurance, we are required to collect copayments and all standard rates of service are applied to any deductibles linked with your plan. Payment is required at the time of the visit.

2...Utilize AFH self-pay rates. AFH offers upwards of a 60% discount (variable with service or testing) off current billed-to-insurance rates. To access this option, patients simply notifies our receptionist prior to the visit. Payment is due date of service. This option can be utilized even if a patient actively has health insurance, such as with high deductible health plans.

3... Enroll in our AFH-Exclusive Plan (AFH-EP). This service must be requested and paid for prior to the next scheduled office visit. Effective 1/1/2025, the annual fee is $1,200.00 and is non refundable.

This package includes, but is not limited to:

- Up to 10 in-clinic office visits (routine, urgent, telemedical, and wellness exams)* ** ***

- All in-office administered medications

- Select lab testing ordered through our office (must be ordered by AFH provider) and all available in-house diagnostic testing.

- Free access to our secure patient portal to view your lab results anywhere

- In-office skilled services (IV therapy, simple surgical procedures, laceration repair, splinting, etc).

This option removes the worry of meeting deductibles linked to office visits, lab testing, and more. It can potentially save up to 95% off traditional insurance-billed costs!

Discounted laboratory testing is also available, both through AFH (for established patients) and through Quest Laboratories in our laboratory service center, located between AFH and Current Chiropractic. To receive this discount, patient accounts must be in good financial standing.

*Specialized examination visits, such as CDL and Sports Physical Examinations are not included, but are offered at traditional self-pay discount rates)

**AFH-EP expires 1 year from the date of purchase.

***Additional visits or services utilized through the end of the contracted term but outside the maximum allowed benefits are billed at current AFH self-pay rates. Payment for services rendered due at time of visit.
